What to Bring
For your appointment at the WIC clinic, please bring:
- Identification, such as:
- Oregon driver's license
- State ID card, a passport
- Work or school ID
- Oregon Health Plan medical ID card
- Current WIC ID card
- Proof of income, such as:
- Food Stamp (SNAP) award letter
- Temporary Assistance to Needy Families (TANF) "Notice of Approval" or most recent "Change Notice" letter
- Current pay stubs
- Unemployment benefit stub
- W2 Forms or income tax return
- Foster Child/ Parent Placement Letter
- Signed letter from employer stating gross earnings
- Food Distribution Program on Indian Reservations (FDPIR)
- Proof of residence, such as:
- Oregon driver's license
- State ID card
- Current utility bill
- Bank statement
- Rent receipt
- WIC ID card, if you already have one. If the appointment is for your child, also bring:
- Proof of identity for her or him, such as:
- Birth certificate
- Immunization record or
- Oregon Health Plan medical ID card
- Her or his immunization record, if you have it
When you make the appointment, ask if you should bring anything else.
At the appointment, WIC staff will:
- Review health history
- Evaluate diet
- Determine eligibility
- Answer questions about nutrition
- Help people find more services
- Provide benefits to buy healthy foods
- Work with people to help them reach their nutrition goals
