The 2018 Lincoln County CHA (PDF). The CHA displays a wide range of data about health indicators and factors that influence health. The CHA will inform the Community Health Improvement Planning (CHIP) process that will set priority areas for community health improvements for Lincoln County for the next five years.

Lincoln County 2019-2024 Community Health Improvement Plan (CHIP)

The Lincoln County 2019-2024 CHIP (PDF) documents months of ongoing collaboration with multiple community partners and stakeholders. This five-year plan is informed by data published in the Lincoln County Community Health Assessment (CHA) in 2018. The plan outlines the top public health priorities identified by community partners, and defines objectives and strategies for each priority that partners have agreed to accomplish utilizing a collective impact framework.

The Lincoln County Community Health Assessment (CHA) is the result of many dedicated hours of collaboration among community partners and agencies, leaders, and local residents. Lincoln County Health and Human Services was the driving force behind this 10-month endeavor to provide an accurate and current overview of Lincoln County's health. The CHA describes a wide array of information about the condition and factors affecting people's health in Lincoln County as well as indicators of health status.

Regional Health Assessment of Linn, Benton, and Lincoln Counties 2015-2020 (PDF)

The 2015 Regional Health Assessment (RHA) is the result of many dedicated hours of research, working in collaboration with community partners and agencies, leaders, and local residents across all three counties. It is a compilation of the individual counties' community health assessments with the addition of information on many topic areas within the CHAs as well as information on systems and networks within the region.

Health is not just about individuals, but includes families, communities, and systems, and is a result of the interaction of complex networks of condition and factors. Health starts long before illness occurs and is impacted by where and how we live, learn, work, play, worship, and age.

The 2016 Lincoln County Public Health Division Annual Report summarizes the programs and services provided in the previous year. In addition, the report includes key data to show improvements in service delivery and health outcomes as well as projects the Lincoln County Public Health Division will be working on in the coming year to improve and modernize our health department.
