The Lincoln County Sheriff's Office Equal Employment Opportunity Plan (EEOP) is required as a condition of certain federal law enforcement grants.

This form is required by Federal Law for the sole purpose of establishing proof of an individual's eligibility for employment in the United States. Along with this form, you are required to show two proofs of identification. The following documents are acceptable:

  • Driver's License and Birth Certificate
  • U.S. Military Card and Social Security Card
